If any situation changes whilst I am engaged in an assignment by Driver Support Services, I will immediately notify Driver Support Services and if appropriate the Client company where I am working. I further understand that Kirivani Ltd (t/a Driver Support Services), will not discriminate in advertising, selecting, offering training or providing benefits and services.     Basic Procedures

    The following basic procedures must be adhered to at all times to prevent any costs being passed to you and any legal action taken against you

    • It is the driver’s responsibility to ensure all loads are securely strapped; failure to do this will result in any cost being passed onto the driver if there is a shot load.

    • If there are more pallets than straps on your trailer, then as a driver you must report this immediately to the transport department. If you leave with less straps than pallets which results in a shot load then you (the driver) will be responsible for cost of restack, repack & re- delivery which could incur an extra cost of fuel.

    • Please ensure you communicate with the transport department at all times, especially after your last drop.

    • Please ensure you do not come back with an empty trailer unless you have been advised to do so. If you (the driver) comes back with an empty trailer without permission, then the cost of loss of earnings from the client will be passed on to the driver.

    • All drivers are to check their vehicles before they leave site to start their shift, if any driver fails to check their vehicle prior to the start of the shift then you (the driver) will be responsible as it is the driver’s duty to report any defects and/or any damages to ensure they cover themselves with our clients and/or with VOSA.

    • Smoking is not allowed at all in any of our clients’ vehicles. Any drivers found smoking will be banned from site and any valet cost will be deducted from the driver.

    • All accidents must be reported to our clients and the agency immediately. Please ensure you take pictures of any and all damage that may have occurred however major or minor.

    By signing this you accept that you have read and understood the above. You also accept that if you (the driver) is at fault for any of the above after acknowledging this then money will be deducted from your wages to cover any claim which falls outside of the Driver’s Negligence Insurance. You also accept that if you (the driver) is at fault for anything falling inside Driver’s Negligence you (the driver) are liable for paying the excess of £750 for claims over this amount. Please ensure the above are adhered to in order to prevent any claim made against Driver Support Services or Kirivani Ltd and to prevent any monies being deducted from you (the driver).

I understand and agree to the passing on of these references, once I have accepted employment, to a future employer.

    I further agree to treat as confidential, any information I receive concerning the business of Kirivani Ltd trading as Driver Support Services or its clients and not to disclose such information in any other way than as directed by Driver Support Services in connection with the business of Kirvani Ltd T/A Driver Support Services in accordance with company policy.

    Criminal Convictions:

    If you have any unspent criminal convictions, please list the offence and their dates below.

    The information you give will be treated in confidence and only taken in to account where, in the reasonable opinion of Driver Support Services the offence is relevant to the post for which you are applying. A conviction will become spent after a “rehabilitation period”.

    A list of these periods are available online and will depend on the sentence received. Certain types of employment and certain professions are exempted from the Rehabilitation of Offenders Act 1974 and in those cases particularly where the employment is sought in relation to positions involving working with children or vulnerable adults, details of all criminal convictions, both spent and unspent, must be given. A list of the types of employment or professions is available.

    Failure to declare a conviction may require us to exclude you from our register and/or terminate an assignment and/or your employment if the offence is not declared but later comes to light.

    DVLA Check Consent

    The DVLA have introduced an online service where all licence holders can view their license and categories on-line. As an employer it is our responsibility to ensure that we are fully compliant with the authorities to ensure we are employing the correct drivers who hold the correct categories and entitlements. We carry out quarterly licence reviews to ensure that our drivers are working within the guidelines at all times and by signing this you authorise Kirivani Ltd T/A Driver Support Services to carry out quarterly licence reviews on your licence online with the DVLA.

    Sign below to confirm that you give Kirivani Ltd T/A Driver Support Services permission to carry out quarterly licence reviews via the DVLA online service whenever required and deemed necessary by the Company.

    Holiday Pay Declaration:

    All entitlement to leave must be taken during the course of the leave year in which it accrues and none may be carried forward to the next year. Our leave year is from Jan 1 st to Dec 31 st. All employees need to request all holiday pay within this period, any holiday requested after the 31 st December will be lost and will not be paid as neither Kirivani Ltd T/A Driver Support Services allow holidays to be rolled over to the following year.

    Company Pension Scheme

    All PAYE employees are entitled to holiday pay. Holiday entitlement is 28 days per year inclusive of bank and public holidays subject to pro-rata, holiday pay is accrued on a daily basis and you must have accrued enough holiday entitlement before you take any leave. Holiday year runs from 1st January to 31st December. Holidays must be claimed during this period, holidays not claimed during this time will result in a loss of holiday pay

    If you wish to book holidays, then you must provide a minimum of 1 weeks’ notice. You can request holiday pay by informing the relevant consultant who will inform the accounts department.

    If your salary is being processed PAYE, then please be advised of the following; to help people save more for their retirement, the government now requires employers to enrol their workers into a workplace pension scheme. This applies to those who aren’t already in one and who:

    • earn over £ 10,600 per year (£ 883 per month)

    • are aged 22 or over; and

    • are under State Pension Age

    • We will therefore be required to enrol you into our pension scheme. The scheme is provided by NEST(National Employment Savings Trust) – The Pension Provider. You can choose to opt out of the scheme if you want to, but if you stay in you will have your own pension pot to access when you retire. Both you and the company (Kirivani Ltd) will pay into it every week and the government will also contribute through tax relief. Your pension belongs to you even if you leave us in the future. If
